The Hampshire County Health Department will be visiting Romney Middle School, Capon Bridge Middle School and Hampshire High School on March 9th and April 17th to administer vaccines for students going into 7th and 12th grades for Fall 2023. These are mandatory to have completed prior to the Fall 2023 school year. Please complete the appropriate form and return to your school nurse if you would like your child to get their vaccine at school on one of the above dates. Thank you!

Please download, print, and complete the appropriate consent form and send to the school with the student.

Immunization Requirements for Students

State law and West Virginia Board of Education also require certain vaccinations for first entry into school and in addition all 6th- and 11th-graders must receive vaccine updates prior to entering 7th and 12th grade.
